Update Stale workflow.

- Change days before stale to 30 from 180.
- Update action version.
- Explicitly specify permissions.
- Remove boilerplate comments.
- Add more details to issue close messages.
- Remove unnecessary debug-only option.
- Exempt any issues or PRs with assignees.
